Progesterone and Epilepsy

Many women with epilepsy experience an exacerbation of their disorder that can be related to their menstrual cycle. This is referred to as catamenial epilepsy and its most common pattern is an increased tendency for seizures just before or at the onset of menstruation. The exact pathophysiology of the seizures is unclear, but the timing of these episodes corresponds to the significant drop in circulating progesterone, which increases the estradiol: progesterone ratio. Estradiol and progesterone have unique properties that act on the nerve cells of the brain: estradiol increases the metabolism and firing rates of neurons, while progesterone has the opposite effect.

These facts led researchers at North Carolina State University to study the effects of neurosteriod replacement therapy on patients with catamenial epilepsy. In a 3-month investigation, natural progesterone was administered on days 15-28 of the menstrual cycle. A 54% to 68% reduction in seizure frequency was reported at the end of the study period. A three year follow up on the 15 patients who continued the use of progesterone reflected a 62% to 74% decrease in the frequency of their seizures. Many further studies and research projects have been performed, with the same results.

The relationship between hormones and seizures can be a vicious circle: not only can hormone levels affect seizures, seizures can affect hormones. Women with temporal lobe seizures have a higher rate of menstrual dysfunction and reproductive disorders than those without epilepsy. The altered function of the temporal lobe, which ultimately controls the process of hormone secretion, provokes hormonal changes that are associated with decreased progesterone. This leads to the imbalance of estrogen to progesterone. Treatment with progesterone normalizes the ratio, and seizures are less frequent. This means temporal lobe controls become more normal, resolving menstrual disorders.

It is important to note that the successful use of progesterone in this treatment is with natural, not synthetic hormone. Synthetic hormones, or progestins, do not have the same effect on reducing epileptic seizures. It is theorized that this is because the metabolite of the natural form, allopregnanalone, is the potent component and that it does not exist in progestins.

Neurosteroids are currently being investigated as promising treatments for mood and anxiety disorders. This shift of awareness should lead to more interest in the influence our hormones have on our health and well being, and to a more wide spread acceptance of using hormones therapeutically.

Six pumps of crème (1 teaspoon) will supply 30 ml of micronized natural progesterone USP with a triple strength herbal complex for adrenal support.
Directions: Two pumps of creme (1 teaspoon) equals 30 mg of micronized natural progesterone USP (Bioidentical) from wild yam plus herbal extracts for adrenal exhaustion.
